| flagged | Adjudicator can’t decide alone — escalating to a senior auditor or asking the submitter for more info. Case stays open; nothing is paid yet. |
| rejected | Adjudicator has high confidence the receipt is forged or invalid. Claim denied. May trigger a fraud investigation per company policy. |
